| At 4:15 PST the intermittent call failure on origination was resolved. The issue was isolated to a single carrier that upon change to their network resolved the issue. |

International Call Routing Changes |
| As you may already be aware, International calling fraud has become a significant problem for both service providers and users of VoIP services. Typically, unauthorized access is gained and then calls are placed to very expensive regions of the world, creating large bills for someone else. In response, Velocity has identified some specific countries that are particularly problematic and has Implemented an immediate, emergency blocking of calls to those countries. This was necessary in order to minimize exposure to the fraudulent use of your VoIP services and to avoid the related charges. If you have International calling capabilities enabled through Velocity Networks and find that legitimate calls are not going through, please contact Velocity Support (800-404-5407 or support@vel.net). Velocity Networks will work with you to get your calls to their intended destination. |

DSL and T1 problem Resolved |
| The problem was identified and resolved, all circuits that were have issues have new been restored. Any circuits that have issues from this point forward, will be worked as an individual issue through our support department. |

| We are having a problem with one of our circuit concentrators in our Los Angeles POP. Some customers be experiening intermittent downtime and slowness. Our engineers are working on the issue, there is not ETA at this time. |

| Maintenance is schedule for one of our T1 circuit routers, 11/02/2011 at 01:00 PST.
We anticipate brief periods of interruption to select T1s while the router is upgraded and rebooted. The maintenance window is 11/02/2011 1:00 to 2:00 am pst. |

Postini login problem |
| The postini login issue has been resolved. All users should be able to log in and check their spam and virus quarantine list. Email filter was not interrupted during the authentication problem. Please contact our support department if you have any problems with your authentication. |

| We have identified a problem with password authentication for our spam and antivirus email solution. We are working on resolving the issue, there is no ETA at this time. |

Intermittent Broadsoft Outage |
| On 10/28/2011 at about midnight PST, a registration flood on Broadsoft caused a fail over between clusters. When the backup cluster attempted to start, the flood caused the system to fail over back to the primary cluster, creating a loop. Once the problem was identified, we were able to stop the registration flood and Broadsoft returned to full functionality. During the outage, some Broadsoft enterprises were unable to make and receive calls. |

SLC POP media routing issue |
| Media routing has been resolved in SLC. The problem was isolated to a media memory leak on the voice switch. Once the problem was identified we were supplied a patch which ultimately resolved the issue. |
